肝脾CT值分析在非酒精性脂肪肝诊断中的应用

Application of Liver-spleen CT Value Analysis in Nonalcoholic Fatty Liver Disease

探讨肝脾CT值分析在非酒精性脂肪肝中诊断及严重程度的关系。对163例临床明确诊断的非酒精性脂肪肝患者,分别根据肝脏CT值、肝脾CT值比值及肝脾CT值差值进行分组。结果三种评价方法对重度非酒精性脂肪肝均有较高的灵敏度及特异度,轻度非酒精性脂肪肝样本中出现28例肝脾CT比值>1,肝脾值差值与肝脾CT值比值在对脂肪肝严重程度上有明显的相关性,肝脏CT值与肝脾CT值比值在对脂肪肝严重程度上没有明显的相关性。肝脾CT值分析在重度NAFLD诊断中有较高的准确性。肝CT值高于脾CT值不能完全排除NAFLD。肝脾CT值差值也可作为NAFLD严重程度量化的参考评定标准,肝脏CT值不宜作为NAFLD严重程度评定标准。

To explore liver-spleen CT value analysis in diagnosis and leison severity in nonalcoholic fatty liver disease （NAFLD）. Among 163 patients with clinical non-alcoholic fatty liver disease,groups were devided according to hepatic CT attenuation,liver-to-spleen CT value ratio and spleen-minus-liver attenuation respectively. All three analysis methods have more reliable Sensitivity and Specificity in severe NAFLD . 28 cases that liver-to-spleen CT value）1 can be demonstrated in the sample of mild NAFLD . The group of liver to-spleen CT value ratio had significant correlation with that of spleen-minus-liver attenuation in the study, and had no correlation with the group of hepatic CT attenuation. Application of liver-spleen CT value analysis is more reliable in severe NAFLD . Cases that liver-to-spleen CT value） 1 cannot be excluded the possibility of NAFLD. Spleen-minus-liver attenuation calculation can be served as quantitative analysis reference standard in NAFLD,and otherwise hepatic CT attenuation analysis is unfit for doing it.
