低温透析对透析中低血压患者血浆脑钠肽影响的观察 被引量:5

摘要 目的:观察低温透析对透析中低血压(IDH)患者血浆脑钠肽(BNP)的影响。方法:选择维持性血液透析6个月以上,近1个月≥30%透析过程中发生IDH 20例,前4周透析温度设定为37℃,后4周透析温度设定为35℃;连续监测血压等血流动力学指标,每周检测末次透析后BNP、生化指标和血常规,共观察8周。结果:(1)低温透析后BNP水平降低幅度显著大于常温透析(P<0.05);(2)低温透析时收缩压、舒张压及平均动脉压显著或非常显著高于常温透析(P<0.05,P<0.01),心率显著低于常温透析(P<0.05);(3)低温透析中症状性及无症状性IDH发生率非常显著低于常温透析(P<0.01);(4)低温或常温透析对血红蛋白、电解质、C-反应蛋白、肌钙蛋白-T、甲状旁腺素水平等均无显著影响。结论:低温透析能显著降低IDH患者血浆BNP水平,有效保护左心室功能。
