吲哚菁绿荧光血管造影在颅内动脉瘤手术中的临床研究

摘要 目的探讨吲哚箐绿荧光血管造影治疗颅内动脉瘤的应用价值。方法回顾性总结10例10个颅内动脉瘤。术中采用吲哚箐绿荧光血管造影,评估动脉瘤夹闭情况以及载瘤动脉的血供情况,根据造影结果必要时调整动脉瘤夹。术后行CT血管显像(CTA)判断动脉瘤夹闭情况。结果术中荧光血管造影发现动脉瘤残颈2例,载瘤动脉狭窄1例,均根据造影结果及时调整动脉瘤夹。术后CTA检查证实动脉瘤夹闭完全。结论显微镜联合吲哚菁绿荧光血管造影是动脉瘤手术中的有用工具,为动脉瘤夹闭术中提供实时信息,及时检查手术效果,可有效减少术后并发症,提高手术的安全性,从而能有效改善患者预后。 Objective To study the application of intraoperative indocyanine green videoangiog- raphy in the surgical treatment of intracranial aneurysms. Methods Ten patients harboring 10 intracranial aneurysms were retrospectively analyzed. All the patients routinely underwent indocyanine green videoangiography during the clipping of aneurysms. According to the result of fluorescent angiography, the clip was adjusted if necessary. Postoperatively, CTA was performed to confirm the complete aneurysm obliteration. Results During the operations, aneuryismal neck remnant was found in two cases, parent arteries stenosis was found in one case. According to the operative findings, the clips were replaced until satisfactory blood flow was restored. Complete clipping and parent bypass vessels were comfirmed by CTA exams. Conclusion Microscope -integrated ICG angiography is a useful tool in aneurysm surgery, which may provides real - time information during aneurysm surgery, and help the operator real - time inspection surgical operation effect. It will play an important role in improving the clinical outcomes of intracranial aneurysm. Thus it can effectively imorove the prognosis of patients.
