面部皮肤癌切除及修复整形94例临床分析

Exploration of reconstructed methods and clinical effects of facial skin cancer 94 cases

目的探讨面部皮肤癌的手术治疗及修复整形的临床效果。方法 94例面部皮肤癌,根据病灶的部位、范围和术中冰冻切片,进行扩大切除后采取游离创缘后拉拢缝合、游离耳后皮片移植和局部皮瓣转移一期修复创面。结果患者皮瓣及皮片全部一期成活,切口愈合良好,外形满意。随访1个月～3年,1例低分化鳞癌术后,发生肺转移,1年后死亡。结论面部皮肤癌宜早期行手术扩大切除术,术中切缘冰冻切片可以保证肿瘤的完全切除。并根据不同情况选用不同方法修复创面,可取得较满意的疗效及预后。

Objective To investigate reconstructed methods and clinical effects of facial skin cancer.Methods Totally 94 patients with facial skin cancer were repaired with different surgical measures,such as direct closure,auricular skin graft and random flaps.All resection margins were checked by intraoperative frozen section.Results All defects with flap and skin graft were closed primarily,the facial appearance and function were satisfied,followed-up for 1 months to 3 years,1 patient with poorly differentiated squamous cell carcinoma died due to the occurrence of lung metastases after one year.Conclusions Patients with facial skin cancer should expand early surgical resection,it＇s important to ensure complete removal of tumor by intraoperative frozen section.Choose different methods according to different circumstances can be achieved more satisfactory results and prognosis.
