强直性脊柱炎全髋关节置换术临床治疗分析

Clinical analysis of ankylosing spondylitis in total hip arthroplasty

目的分析强直性脊柱炎(AS)患者行髋关节置换术(THA)的临床经验。方法全髋关节置换术(THA)30例,术前髋关节总活动度0°～100°,平均31.4°。其中双髋同时置换20例,分次手术3例,术后随访时间12～24个月,平均20.5个月。结果所有患者术后随访期内均未发生大血管损伤、关节脱位、感染等并发症,髋痛完全缓解率90.0%,术后髋关活动度明显提高。Harris评分75～92分,平均90.5分,优良率为90.0%(45/50)。术后患者满意度达100%。结论由于强直性脊柱炎本身的病理特征,依据患者的病情及年龄选择手术时机和合适假体。

Objective To analyze the clinical experience of ankylosing spondylitis（AS） patients with hip arthroplasty（THA） .Methods The total hip arthroplasty（THA） 30 cases,preoperative total hip activity 0°～100°,an average of 31.4°.One double hip replacement in 20 cases,sub-second operation 3 cases,were followed up from 12 to 24 months,average 20.5 months.Results All patients were followed up during the period were not a major vessel injury,joint dislocation,infection and other complications,hip pain,complete remission rate of 90.0%,postoperative hip related activity was significantly increased.Harris scores 75 to 92,average 90.5 points,the fine rate was 90.0%（45/50） .Postoperative patient satisfaction was 100%.Conclusions Because of ankylosing spondylitis the pathological features of its own,based on the patient＇s condition and age appropriate selection of operative time and the prosthesis.
