Results analysis of milk hepatitis B virus DNA tests for puerpera infected by or carrying hepatitis B virus

目的:研究乙肝病毒(HBV)感染或携带产妇乳汁中的HBV-DNA含量,探讨HBV携带产妇乳汁的传染性。方法:对86例HBV携带产妇的血清和乳汁进行HBV检测,根据产妇HBV血清标志物分为大三阳组(A组23例)、小三阳组(B组34例)和双抗阳性组(C组29例),分析各组血清中HBV含量与乳汁中HBV-DNA含量的关系。结果:大三阳组的乳汁HBV-DNA阳性率最高(78.3%),双抗阳性组的HBV-DNA阳性率最低(10.3%),经检验大三阳组的产妇乳汁中HBV-DNA阳性率显著高于小三阳组和双抗阳性组(χ2=26.74,P<0.001)。当HBV-DNA≥106 copies/ml时,乳汁中才能检出HBV-DNA阳性。结论:产妇血清中HBV水平的高低与乳汁中HBV成正相关关系。血清中HBV-DNA检测结果小于106 copies/ml时,乳汁中未检测出HBV-DNA阳性,此时建议对婴儿采取一定的免疫预防措施,可继续母乳喂养。

Objective： To study the content of hepatitis B virus DNA（HBV-DNA） in the milk of puerpera infected by or carrying HBV and explore the infectivity of the milk of the HBV carrying puerpera.Methods： 86 patients carrying HBV were given HVB tests in the serum and milk.The patients were divided into the large three positive group（23 patients in group A）,the small three positive group（34 patients in group B） and double antibody positive group（29 patients in group C） according to the HBV serum markers.Then the relationship between serum HBV content and milk HBV-DNA content were analyzed.Results： The milk HBV-DNA positive rate of the large three positive group was the highest（78.3%） while that of the double antibody positive group（10.3%） was the lowest.The milk HBV-DNA positive rate of the large three positive group was significantly higher than that of the small three positive group and of the double antibody positive group（χ2=26.74,P〈0.001）.Only when serum HBV-DNA≥106 copies/ml,the milk can be detected as HBV-DNA positive.Conclusion： The serum level of HBV is in positive correlation to the milk level of HBV.For serum HBV-DNA≤106 copies/ml,the milk is not detected as HBV-DNA positive,so it is recommended to take certain preventive immune measures to continue breastfeeding the babies.
