The Expression of p53 and Bcl-2 Proteins in Colorectal Cancer and the Correlation with Prognosis

摘要 目的探讨p53、Bcl-2蛋白在结直肠癌组织中的表达及其与预后的关系。方法应用免疫组化SABC法检测120例手术切除的结直肠癌肿瘤组织中p53、Bcl-2蛋白的表达,并对所有手术后病例进行随访,分析5年生存率与p53及Bcl-2阳性表达的关系。结果结直肠癌肿瘤组织中p53、Bcl-2蛋白表达阳性率分别为63.3%、46.7%。p53、Bcl-2蛋白阳性表达均与患者年龄、性别、肿瘤Dukes分期、生长部位、淋巴结是否转移无关(P>0.05),与肿瘤分化程度有关(P<0.05);Spearman相关性分析表明,在结直肠癌肿瘤组织中p53和Bcl-2的表达呈负相关;患者术后的5年生存率与Bcl-2蛋白表达呈正相关(P<0.01);与p53表达呈负相关(P<0.05)。结论 p53在结直肠癌组织中高表达,而Bcl-2蛋白低表达,且两者成负相关。p53和Bcl-2与患者术后5年生存率均有关,提示检测p53和Bcl-2的表达,对临床判断术后患者的预后有指导意义。 Objective To investigate the expression of p53 and Bcl-2 proteins in colorectal cancer and to elucidate its relationship with prognosis.Methods The expression of p53 and Bcl-2 was detected in 120 cases of surgical resection colorectal cancer tissue by using SABC immunohistochemical methods.Furthermore,The follow up study was done for all of the operated and to analyze relation between 5-year survival rate of patients with the expression of p53 and Bcl-2 genes in colorectal cancer.Results The positive rate of p53 and Bcl-2 in colorectal carcinoma tissues was 63.3% and 46.7%,respectively.The expression of p53 and Bcl-2 had no relationship with age,sex,the Dukes stage,the position of tumor and the lymph node metastasis(P〉0.05);the positive expression rate of p53 and Bcl-2 was correlated with the differentiation of tumor tissues(P〈0.05).Furthermore,the expression of p53 and Bcl-2 in tumor tissues had negative correlation with each other by Spearman's rank correlation.Follow-up study indicated that the expression of Bcl-2 had positive correlation with the 5-year survival rate(P〈0.01);in contrast,p53 had negative correlation with the 5-year survival rate of postoperative patients(P〈0.05).Conclusion p53 is highly expressed but Bcl-2 is low expressed in colorectal carcinoma.The expression of p53 and bcl-2 had relationship with the 5-year survival rate of postoperative patients.The detection of p53 and bcl-2 expression may be useful in the prognosis of the postoperative colorectal patients.
