摘要 目的:探讨节段分析法联合九平面连续扫查法在胎儿心脏超声检查中的临床价值。方法:回顾性分析本院2009年3月~2010年10月,16488例在本院做中、晚期妊娠系统胎儿超声检查(Ⅲ级产前超声检查)的孕妇的临床资料。分析包括心房、心室、大血管的辨认及心房心室、心室大血管连接的判断。结果:本研究中16488例中、晚期妊娠胎儿经产前超声检查及尸解发现的主要心脏及血管异常124例,83例超声检查有明显的心内结构异常,尸解发现均为复杂性和严重性先天性心脏病。结论:①节段性分析法在胎儿心脏的超声检查中是必需的,它能帮助发现许多胎儿心脏的结构异常,这些多数是严重的、复杂性的先天性心脏病,对于妊娠筛选、分娩处理及产后干预有重要意义。②节段性分析法对单纯性先天性心脏病有局限性。联合九平面连续扫查法能显示心脏的许多解剖结构,使胎儿心脏、血管结构、血流和功能的确认得到清晰显示。 Objective:To investigate the clinical value of segmental analysis combined nine consecutive scanning plane methods in the fetal echocardiography.Methods:Did retrospective analyzed to the clinical data of 16 488 pregnant women in late pregnancy fetal ultrasound system(Ⅲ level of prenatal ultrasound examination) in our hospital from March 2009 to October 2010.Analyzed the judgment of connection including atrium,ventricle,identify the great vessels and heart ventricle,ventricular great vessel.Results:In this study,16 488 cases of middle and late pregnancy by prenatal ultrasound and fetal autopsy were found that there were 124 cases with the major cardiac and vascular abnormalities,83 patients had significant structural heart abnormalities by ultrasonography,autopsy found that both with complexity and severity of congenital heart disease.Conclusion:① segmental analysis in fetal cardiac ultrasound examination is necessary,it can help find many fetal structural abnormalities of the heart,they are most serious,complex congenital heart disease,it is important to screen for pregnancy,childbirth and postnatal intervention treatment.② segmental analysis of simple congenital heart disease have limitations.Joint nine consecutive scanning plane method can show the heart of many of the anatomical structure,making the fetal heart,vascular structure,blood flow and function of the confirmation can be clearly displayed.
