2009~2010年我院病原菌分布及耐药性分析

摘要 目的了解2009~2010年临床分离的病原菌分布及耐药性,为临床合理使用抗菌药物提供依据。方法回顾性分析2009~2010年所分离的病原菌及药物敏感试验结果。结果共分离出病原菌4038株,痰液分离率最高(84.97%);革兰阴性菌3043株(75.36%),革兰阳性球菌260株(6.44%),真菌735株(18.20%)。革兰阴性菌居前5位的依次是肺炎克雷伯杆菌、白假丝酵母菌、鲍曼不动杆菌、铜绿假单胞菌、大肠埃希菌;革兰阳性菌以金黄色葡萄球菌和表皮葡萄球菌为主;真菌以白假丝酵母菌为主;革兰阴性菌对碳氢酶烯类、喹诺酮类、氨基糖苷类等抗菌药物敏感;肺炎克雷伯杆菌、大肠埃希杆菌产ESBL的分别为13.56%、25.62%。MRSA、MRSE分别为36.68%、44.44%;未发现耐万古霉素的葡萄球菌和肠球菌。结论加强耐药菌的监测,对临床合理用药具有重大意义。 Objective To investigate the pathogenic distribution and antimicrobial resistance of clinical isolated strains in our hospital from 2009 to 2010 and provide the rational information to use antibiotics reasonably.Methods All pathogens isolated from patients in our hospital from 2009 to 2010 and drug susceptibility results were retrospectively analyzed.Results Totally 4038 strains of pathogenic bacteria were isolated,Sputum was the major specimen(84.97%);in which 3043 strains of Gram-negative bacteria(75.36%),260 strains of Gram-positive bacteria(6.44%),and 735 strains of fungi(18.20%).The top five isolates of Gram-negative bacteria were Klebsiella pneumoniae,Candida albicansthe,Acinetobacter baumannii,Pseudomonas aeruginosa,Escherichia coli;Staphyococcus epidermidis and S.aureus were the main pathogens of Gram-positive bacteria.Candida albicansthe was the main pathogen of fungi.Gram-negative bacteria were sensitive to imipenem,meropenem,quinolones,aminoglycosides.Klebsiella pneumoniae and E.coli produced ESBLs rates were 13.56% and 25.62%.The prevalence of MRSA and MRSE were 36.68% and 44.44%.No vancomycin resistant Staphy lococcus or Enterococcus was detected.Conclusion Strengthening continuous surveillance of drug resistance of pathogens can be useful for antibiotic usage reasonably.
