粉尘螨滴剂治疗粉尘螨致敏过敏性鼻炎和哮喘的疗效及其作用机制 被引量:17

摘要 目的探讨舌下特异性免疫疗法(SLIT)治疗过敏性鼻炎和哮喘的临床疗效及其作用机制。方法选择30例对粉尘螨过敏的过敏性鼻炎和轻度哮喘患儿(观察组),采用粉尘螨滴剂(畅迪)治疗24周;另选25例同龄健康儿童作为正常对照组。应用双抗体夹心ELISA法检测两组血清IL-17、IL-10、特异性IgE(SIgE)、特异性IgG4(SIgG4);测定观察组治疗前后外周血嗜酸性粒细胞(Eos)计数、肺功能/气道阻力,以及鼻炎和哮喘症状评分、症状总评分。结果与正常对照组及同组治疗前比较,观察组治疗后血清IL-17下降,IL-10、SIgG4升高,外周血Eos计数下降,第1秒用力呼气容积占预计值百分率升高,气道阻力下降(P<0.01或<0.05)。观察组治疗前后血清IL-10与SIgG4水平均呈正相关,与症状总评分均呈负相关(P均<0.01);有效率为93.3%%。结论 SLIT是儿童呼吸道变应性疾病安全、有效的治疗方法,其可能通过抑制Th17细胞分化、相关促炎症细胞因子表达和Th2细胞因子产生,增强调节性T细胞活性,调节机体免疫状态,诱导免疫耐受。
