老年基底动脉尖综合征患者的临床和磁共振成像特点分析

The clinical and MRI features of top of the basilar artery syndrome in elderly population

目的探讨老年基底动脉尖综合征(TOBS)患者的临床和MRI特点,为临床提供依据。方法选择TOBS老年患者28例,采用TOAST分型和美国国立卫生院卒中量表评分,分析临床和MRI特点。结果老年TOBS患者以眩晕(82%)、意识障碍(68%)多见。MRI显示,中脑梗死占64%,脑桥占36%,丘脑占43%,枕叶占39%,小脑半球占68%。结论掌握老年TOBS患者临床及MRI特点,为进一步诊断、治疗提供依据。

Objective To explore the clinical and MRI features of top of the basilar artery syndrome（TOBS） in elderly population. Methods Twenty-eight TOBS patients who were above 65 years old were selected,TOAST classification and NIHSS were adopted to investigate their clinical and MRI features. Results The clinical manifestations were diversified. Vertigo and consciousness disorder were the most common features, ocular motility disorders, hemianopsia and ataxia were also frequent characteristics. As for the distributions of MRI lesions,midbrain infarction accounted for 64%, pons, thalamus, occipital lobe and cerebellar hemisphere accounted for 36% ,43% ,39% and 68% ,respectively. These features were different from those in ordinary population. Conclusion TOBS among elderly population has its unique characteristics, mastering these characteristics is conducive to the correct understanding of such disease and the further diagnosis and therapy.
