疏肝和胃降逆汤治疗胃食管反流病临床观察

Clinical Observation on Shugan Hewei Jiangni Decoction in Treating Patients with Gastroesophageal Reflux Disease

目的:观察中药疏肝和胃降逆汤治疗胃食管反流病(GERD)的临床疗效。方法:67例来诊的符合纳入标准的胃食管反流病患者,按动态随机Taves最小化法分为两组。治疗组33例,口服疏肝和胃降逆汤,每天1剂;对照组34例,口服奥美拉唑20mg,每日1次,莫沙必利5mg,每日3次;两组疗程均为8周。于治疗后进行临床症状疗效、胃镜下EE分级和复发情况评价。结果:疏肝和胃降逆汤在改善临床综合症状和胃镜下黏膜损害方面与奥美拉唑、莫沙必利联合用药的疗效相似,但对于胸痛和嗳气症状的缓解显示出较明显的优势,其有效率为84.2%和91.7%,分别与对照组的47.1%和65.2%比较,差异均有统计学意义(P<0.05),且远期(停药6个月后)复发率较低(与对照组比较,P<0.05)。结论:疏肝和胃降逆汤治疗GERD的疗效较好。

Objective：To observe the therapeutic efficacy of Shugan Hewei Jiangni Decoction （SHJD） in treating gastroesophageal reflux disease （GERD）. Methods：Sixty -seven GERD patients conformed with the inclusive criteria were randomly divided into the treated group（33 patients）and the control group （34 patients）. Patients in the treated group received SHJD orally, one dose per day,while those in the control group received Omeprazole,20 mg once per day, and Mosapride,5mg three times per day. The therapeutic course was 8 weeks. The efficacy on clinical symptoms, EE grades and scores determined by gastroscope and the condition of recurrence were evaluated. Results ： The therapeutic effect of SHJD on GERD was similar to that of Omeprazole and Mosapride in aspects of improving comprehensive clinical symptoms and curing esophageal mucous inflammation （under gastroscope）. But the cure rate of chest pain and belching in treated group was obviously higher than that in the control group （P 〈 0. 05 ）, while the recurrence in the SHJD group was obviously lower than that in the control group in 6 and 12 months after withdrawal of treatment（ P 〈 0. 05 ）. Conclusion：SHJD showed favorable therapeutic efficacy in treating GERD.
