米非司酮治疗更年期功能失调性子宫出血的疗效观察

Efficacy observation of Mifepristone in the treatment of climacteric dysfunctional uterine bleeding

目的：分析米非司酮治疗更年期功能失调性子宫出血的疗效，为临床治疗提供依据。方法：选取2007年1月～2010年12月我院收治更年期功能失调性子宫出血患者28例，随机分为观察组和对照组，每组14例。观察组口服米非司酮治疗，对照组采用丙酸睾丸酮联合甲基睾丸素治疗，观察两组治疗后各类激素变化情况、不良反应及治疗效果。结果：观察组有效率为100％（14／14），对照组有效率为71．43％（10／14），两组比较，差异有高度统计学意义（P〈0．01）。两组治疗后卵泡刺激素（FSH）、黄体生成素（LH）、雌二醇（E2）和孕酮（P）水平均低于同组治疗前水平，差异有统计学意义（P〈0．05）；但治疗前后泌乳素（PRL）和雄激素（T）水平比较，差异无统计学意义（P〉0．05）。观察组治疗后FSH、LH、E2和P水平均低于对照组，差异有统计学意义（P〈0．05），且观察组治疗后各类激素降低水平较对照组显著。观察组发生不良反应3例，对照组发生5例。结论：米非司酮治疗更年期功能失调性子宫出血较传统治疗方法更方便有效，安全性好，不良反应小。值得临床推广使用。

Objective： To explore the efficacy of Mifepristone in the treatment of climacteric dysfunctional uterine bleed- ing, in order to provide basis for clinical treatment. Methods： 28 cases of dysfunctional uterine bleeding patients in our hospital from January 2007 to December 2010 were randomly divided into two groups, each 14 cases. Observation group was given Mifepristone, control group was given Testosterone Propionate and Methyhestosterone, compared the changes of hormone, side reaction and clinical effect. Results： The effective rate of observation group was 100% （14/14）, the effective rate of control group was 71.43%（10/14）, the difference was statistically significant（P〈0.01）. After treatment, FSH, LH, E2 and P of two groups were significantly lower than before （P〈0.05）; before and after treatment, PRL and T were not significant dif- ferent （PM）.05）. After treatment, FSH, LH, E2 and P of observation group was significantly lower than control group （P〈0.05）. There were 3 cases of side reactions in observation group and 5 cases in control group. Conclusion： The clinical effect of Mifepristone is obvious, security, with less adverse reactions and it is worthy of clinical promotion and application.
