0.5%盐酸布比卡因重比重液腰麻在剖宫产手术中的应用

0.5% of bupivacaine hydrochloride proportion liquid application of spinal anaesthesia in caesarean operation

目的探讨0.5%布比卡因重比重液腰麻进行剖宫产手术的效能与安全性。方法将160例患者随机分为A、B两组,A组采用0.5%布比卡因重比重液进行腰麻,取腰麻常规体位,抽取0.75%布比卡因2ml、50%葡萄糖液0.5ml、灭菌注射用水0.5ml配制成0.5%重比重液进行腰麻。平卧位后如果BP<90mmHg将手术床左斜30°,并静脉给予麻黄碱15mg。B组采用传统连续硬膜外麻醉。结果 A组较B组用药量小,卧位后至开始切皮时间及感觉阻滞起效时间比较差异均有统计学意义(P<0.01)。结论 0.5%布比卡因重比重液腰麻用于剖宫产手术操作简单、诱导快捷,用药量少,毒性低、阻滞效果确切、肌松良好、并发症少,血流动力学改变机体能够有效代偿,对母婴均无不良反应,可作为剖宫产手术一种较佳的麻醉方法。

Objective Discussion on the 0.5% proportion of bupivacaine in spinal anesthesia for Cesarean surgery effectiveness and security.Methods 160 patients by randomly divided into A group and B group,A group with a 0.5% proportion of bupivacaine for spinal fluid,take general spinal posture,a 0.75% bupivacaine 2 ml,50% glucose liquid 0.5ml,sterile water for injection 0.5 ml preparation to 0.5% proportion of heavy liquid for spinal anesthesia.If BP90 mm Hg after prostrate surgery bed left-inclined 30°degrees and intravenous ephedrine given 15 mg.B traditional continuous epidural anesthesia.Results A group less than group b and so the amount,after lying position to begin feeling skin incision time and block of onset time is a very notable（P〈0.01）.Conclusion 0.5% proportion of bupivacaine in spinal anesthesia for Cesarean surgery induced by simple operation,fast,less medication,low toxicity,block the effect of,well,less complications of muscle relaxant,hemodynamic change block effective compensation,on mother and infant are no adverse effects,can be used as anesthesia for caesarean operation a better method.
