摘要
目的:研制开发腹主动脉瘤腔内隔绝术国产化器具。方法:以镍钛合金丝编织成Z形支架,外面包裹国产真丝超薄人造血管,装入导鞘制成腔内隔绝系统;以牛颈静脉间置法制作保留腰动脉的犬腹主动脉瘤模型;X线监视下将腔内隔绝系统经犬股动脉导入动脉瘤腔内,释放后将移植物固定在动脉瘤两端正常血管壁上,使动脉瘤与血流隔绝。结果:10只模型犬接受手术,成功9 只。术后即时检查,移植物与宿主动脉结合紧密,没有移位、内漏等并发症发生。1只因动脉破裂死亡。结论:国产腔内隔绝系统的质量、弹性、顺应性、实用性均满意,基本可以替代进口产品而应用于腹主动脉瘤腔内隔绝术。
Objective: To develop domestic apparatus for the treatment of abdominal aortic aneurysms by the method of intravascular graft exclusion. Methods:Ni Ti alloy wire was shaped into Z shaped stents, rapped with domestic super thin silk dacron graft. The complex was inserted into plastic sheath. Canine abdominal aortic aneurysm model was made by inserting a segment of bovine jugular vein. Monitored with X ray, the apparatus was inserted into the lumen of the aneurysm via femoral artery, and then the complex was deployed and anchored to the nearby normal vessel wall. Thus, the aneurysms were excluded from the blood flow. Results: Ten animals received endovascular graft exclusion. Nine cases were successful. One were killed immediately after deployment. The stent graft complex was found combined tightly with the vessel wall. No mismatch or endoleak. One died due to rupture of aorta. Conclusion: The quality, elasticity, compliance and feasibilty of our domestic appratus is satisfactory. It can be used in the treatment of abdominal aortic aneurysm by the method of intravascular graft stent.
