TNF-α抑制剂对破裂型腰椎间盘突出重吸收影响的实验研究 被引量:12

The influence of TNF-α inhibitor in the spontaneous resorption of ruptured lumbar disc herniation: An experimental study

摘要 目的研究TNF-α抑制剂(益赛普)对破裂型腰椎间盘突出重吸收的影响作用。方法将42只雄性SD大鼠在适应性喂养2周后随机分为空白组和药物组。空白组分两种模型:随机分为模型组Ⅰ(椎间盘不刺破组)、模型组Ⅱ(椎间盘刺破组);药物组(TNF-α抑制剂组)给以益赛普注射。动物造模成功后给药4周后处死,所取得的大鼠椎间盘进行TNF-α免疫组化染色和流失细胞仪检测巨噬细胞的含量。结果椎间盘刺破组TNF-α免疫组化染色阳性表达率极高,而椎间盘不刺破组阳性表达率较少,药物组(TNF-α抑制剂组)阳性表达极少;各组都有巨噬细胞的阳性表达,但椎间盘刺破组巨噬细胞的阳性表达率最高。结论破裂型椎间盘突出后容易重吸收,而TNF-α抑制剂减轻突出椎间盘内的炎性反应程度,不利于坏死组织的清除与重吸收。 Objective To investigate the influence of TNF-α inhibitor(Etanercept)in the spontaneous resorption of ruptured lumbar disc herniation.Methods forty-two male SD rats after two weeks of feeding adaptation were randomly divided into drug group and non-drug group.non-drug group were randomly divided into model group Ⅰ(Disc not puncture group)、model group Ⅱ(Disc puncture group);drug group:(TNF-α inhibitor group)injected with etanercept.All animal were sacrificed after the success of animal model after administration for 4weeks.Intervertebral discs in rats made to do TNF-α immunohistochemistry and detected the loss of cell content of macrophages.Results Disc punctured group TNF-α expression by immunohistochemical staining high rate,while disc not puncture group positive rate less,TNF-α inhibitor group have little positive expression;All group has a positive expression of macrophage,but disc puncture group,macrophages positive expression was the highest.Conclusion Ruptured lumbar disc herniation is easily resorption.However,TNF-α inhibitor reduce the inflammatory response within the herniated disc level,is not conducive to the removal of necrotic tissue and resorption.
