预输注右旋美托咪定抑制双腔支气管插管心血管反应的观察 被引量:8

摘要 目的评价预输注不同剂量右旋美托咪定对双腔支气管插管时心血管反应影响。方法胸外科择期手术需行双腔支气管插管患者90例,年龄18—61岁,体质量45—71kg,ASAⅠ~Ⅱ级。分为三组(n=30):生理盐水对照组(A组)、右旋美托咪定0.25μg/kg组(B组)和右旋美托咪定0.5μg/kg组(C组)。麻醉诱导依次静注咪唑安定0.03—0.05mg/kg、舒芬太尼0.25μg/kg、依托咪酯0.2mg/kg、罗库溴铵0.8mg/kg。分别记录麻醉诱导前(T0)、插管前(T1)、插管后即刻(12)、插管后1min(T3)、3min(T4)、10min(T5)时收缩压(SBP)、舒张压(DBP)、心率(HR)。结果与A组比较,B组和C组在插管后的收缩压(SBP)、舒张压(DBP)、心率(HR)均更平稳,但C组在插管前后的显著心率过缓发生率高于B组(P〈0.05)。结论诱导前15min静脉注射DEX0.25μg/kg能安全有效的控制双腔支气管插管反应,保持血流动力学稳定。
