尼尔雌醇联合盐酸氟西汀治疗绝经期妇女抑郁症的疗效观察

Efficacy Observation of Hydrochloride in the Treatment of Menopausal Depression

目的:观察尼尔雌醇联合盐酸氟西汀治疗绝经期妇女抑郁症的疗效。方法:将147例绝经期抑郁症妇女随机分为3组,联合组50例,给予尼尔雌醇,每2周1次,每次1mg,同时每日晨服盐酸氟西汀,每次20mg;尼尔雌醇组50例,给予尼尔雌醇每2周1次,每次1mg;氟西汀组47例,每日晨服盐酸氟西汀,每次20mg。3组疗程均为12周。采用Hamilton抑郁量表(HAMD)评价抑郁严重程度,采用Kupperman评分指数(KMI)评价绝经期症状。结果:治疗后3组HAMD总分均下降,差异有统计学意义(P<0.001),联用组比尼尔雌醇组下降明显,差异有统计学意义(P<0.01)。治疗后3组KMI评分指数总分均下降,差异有统计学意义(P<0.01),但联用组和尼尔雌醇组与氟西汀组比较减分率更高,差异有统计学意义(P<0.01)。3组均未见严重不良反应发生。结论:尼尔雌醇联合盐酸氟西汀治疗发生于绝经期的抑郁症,可以显著提高疗效,且副作用小。

OBJECTIVE：To observe the therapeutic efficacy of nilestriol combined with fluoxetine hydrochloride in the treatment of menopausal depression. METHODS： 147 female with menopausal depression were randomly divided into 3 groups. 50 cases in combination group received nilestriol 1 mg once every 2 weeks and fluoxetine hydrochloride 20 mg once a day in the morning. 50 cases in nilestriol group received nilestriol 1 mg once every 2 weeks. 47 cases in fluoxetine group received fluoxetine 20 mg once a day in the morning. The treatment course lasted for 12 weeks. After treatment, the degree of menopausal depression was evaluated with Hamilton depression score （HAMD） and Kupperman menopausal index （KMI） was used to evaluate symptom in menopausal period. RESULTS： Before and after treatment, HAMD score of combination group, nilestriol group and fluoxetine group decreased. There was significant difference （P0.001）. There was significant difference in the decrease of HAMD score between combination group and nilestriol group （P0.01）.The KMI score in 3 groups before and after treatment decreased, there was significant difference （P0.01）. The decrease of KMI score in combination group and nilestriol group was higher than in fluoxetine group, there was significant difference （P0.01）. There were no adverse drug reactions found in 3 groups. CONCLUSION： Nilestriol combined with fluoxetine hydrochloride in the treatment of menopausal depression can improve therapeutic efficacy significantly with little adverse drug reactions.
