非酒精性脂肪肝患者铜营养状况及其与血脂的关系

Observational Research on Copper Nutrition and Blood Lipid among Patients with Nonalcoholic Fatty Liver Disease

摘要 目的了解非酒精性脂肪肝(NAFLD)患者铜营养代谢水平及其与血脂的关系,为NAFLD的防治提供科学依据。方法采用1:1配对病例-对照研究方法,自行设计调查问卷收集150例非酒精性脂肪肝患者和150例正常对照的基本信息及四种含铜丰富食品的摄入频率,并检测其血清铜含量及血脂各指标,对血铜与血脂等指标进行线性相关分析。结果非酒精性脂肪肝组四种含铜丰富食品的摄入频率和对照组间差异无显著性,但血清铜(1.07±0.25)mg/L高于对照组(1.01±0.22)mg/L,差异有统计学意义(t=3.12,P=0.002)。非酒精性脂肪肝患者总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)高于对照组,高密度脂蛋白胆固醇(HDL-C)低于对照组。相关分析发现,控制年龄和性别后,血清铜与TC、LDL-C呈正相关,与HDL-C呈负相关。结论非酒精性脂肪肝患者存在铜营养代谢异常,其血清铜升高与血脂紊乱有关,而与四种铜丰富食品的摄入频率关系不明显。 Objective To evaluate the levels of copper nutrition and the relationship with lipid in patients with nonalcoholic fatty liver disease (NAFLD). Methods 1:1 matched case-control study was conducted among 150 patients with NAFLD and 150 healthy controls. A self-designed questionnaire was used to collect their general information and intake frequency of 4 kinds of food that was rich in copper. Serum copper and blood lipid were detected and then compared by t-test, and correlation analysis was used to explore the blood lipid's influence on serum copper. Results There was no statistical difference in the intake frequency of 4 kinds of copper-rich food between the two groups. Serum copper of NAFLD patients (1.07±0.25) mg/L was significantly higher than that of healthy controls (1.01 ±0.22) mg/L (t=3.12, P=0.002). Compared with the controls, the levels of TC, TG and LDL-C were higher, but the level of HDL-C was lower in patients. Correlation analysis revealed that serum copper was significant positive correlated with TC and LDL-C, and was significant negative correlated with HDL-C. Conclusion Serum copper and lipid metabolism disorder exist in NAFLD patients, and serum copper relates with blood lipid metabolism disorder, but doesn't relate with the intake of 4 kinds of copper-rich food.
