中孕中期胎儿动脉导管参考值范围的建立及其意义

Establishment and value of reference range for fetal ductus arteriosus during mid-second trimester

目的建立二维心脏超声在中孕中期(20周~23+6周)胎儿动脉导管(ductus arteriosus,DA)内径的参考值范围。方法 2009年1月至2010年5月期间在复旦大学附属妇产科医院进行产前筛查的单胎孕妇4 456例,同时进行胎儿心脏超声,测量DA内径,统计各孕周DA内径参考值范围。结果 4 456例中孕中期胎儿中,4 403例DA显示清晰,DA的显示率为98.81%;各孕周DA测量数据呈正态分布,20周~23+6周的均数为(2.27±0.42)mm^(2.53±0.33)mm;第50百分位数为2.3~2.5 mm。结论中孕中期能够观察到胎儿DA,并能进行管径测量。DA内径参考值的建立可能对早期发现DA异常及DA相关疾病有所帮助。

Objective To establish reference range for the diameter of fetal ductus arteriosus（DA） during mid-second trimester.Methods Two dimensional echocardiography was performed at 20 to 23＋6 weeks in 4 456 singleton fetuses from Jan.2009 to May 2010.Inner diameter of DA was measured.Results DA was displayed clearly in 4 403 out of 4 456 cases,and the display rate of DA was 98.81%.The measured values of DA diameter was normally distributed and increased with gestational age.The mean value of DA diameter was（2.27±0.42）mm to（2.53±0.33）mm,and the 50th percentile was 2.3 to 2.5 mm at 20 to 23＋6 weeks.Conclusions It is possible to observe DA and measure its diameter during mid-second trimester.The reference range for DA diameter in fetus may be helpful for early diagnosis of DA-related heart disease.
