摘要
目的:评价周围神经损伤后,神经吻合口局部应用几丁糖-胶原-二丙酸倍他米松缓释复合膜抑制疤痕增生、促进神经再生、提高神经功能恢复的作用。方法:成年雄性SD大鼠36只,胫神经切断术后随机分为3组,每组12只,A组仅给予单纯缝合,B组给予单纯缝合+吻合口包裹聚-DL-乳酸膜,C组给予单纯缝合+吻合口包裹几丁糖-胶原-二丙酸倍他米松膜。术后4、8及12周观察伤口愈合情况,检测运动神经传导速度和动作电位波幅,免疫荧光法检测神经丝蛋白(NF)的表达,观察神经纤维再生情况。结果:C组神经吻合口局部疤痕增生和粘连程度较其它组明显减轻;再生神经纤维较直,排列结构较整齐;12周后,复合肌肉动作电位(CMAP)高于其它2组(P<0.05)。结论:几丁糖-胶原-二丙酸倍他米松缓释复合膜能有效防止周围神经修复后局部疤痕增生,促进神经的再生。
Objective: To investigate the effects of chitolsan-collagen betamethasone dipropionate film on scarring and regeneration of peripheral nerves in rats following injuries.Methods: Thirty-six SD rats were operated to dissect the right tibial nerves,which were randomly divided into three groups.The nerves in group A were not treated with biological membrane,in the group B with poly-DL-lactic acid(PDLLA) membrane,and group C with chitolsan-collagen betamethasone dipropionate film.The operated SD rats were repaired and fed for the consequent assessment.Histomorphologic nerve analysis,electrophysiologic studies and immunofluorescence detection for neurofilament(NF) were performed at 4,8 and 12 weeks after the surgery.Results: The nerves in the group C demonstrated a significant reduction in perineural scars,an increase in compound muscle action potential(CMAP) and more regular neuronal fibers construction than the other two groups.Conclusion: Chitolsan-collagen betamethasone dipropionate film shows effectiveness in preventing perineural scars formation and in enhancing peripheral nerves regeneration.
