FICE对Barret食管并肠化诊断价值的研究 被引量:2

摘要 目的探讨FICE在早期发现Barret食管(BE)中的肠上皮化生(SIM)等癌前病变中的作用。方法选择2010.1-2010.12共50例经胃镜检查诊断为BE患者,分别采用普通模式、FICE模式对病变范围、腺管开口分型及毛细血管形态观察,对病变作出诊断,并与病理组织学诊断相比较。结果在对鳞柱状上皮交界的病变轮廓,腺管开口形态及毛细血管形态观察中,FICE优于普通内镜。FICE模式下通过Endo分型,检出SIM的准确性,敏感性,特异性分别为92%,86%,94%。结论 FICE可清楚观察BE粘膜病变轮廓,腺管开口形态及毛细血管结构,操作转换简便,对BE食管进行靶向活检具有良好的临床实用价值。 【Objective】 To detect role of Fuji Intelligent Chromo Endoscopy in early diagnosis of precancerous lesion of specialized intestinal metaplasia in Barrett,s esophagus.【Methods】 From January to December 2010,50endoscopic BE patients diagnosed in our hospital were enrolled.They were observed with common endoscopy,then Fuji Intelligent Chromo Endoscopy.The operative endoscopist observed the BE pit pattern by FICE and biopsy in suspicious area to identify special intestine metaplasia.【Results】 The observation of squamous-columnar epithelium,pit pattern and epithelium capillary by FICE was much clearer than by common endoscopy.The accuracy,sensitivity and specificity of diagnsis of specialized intestinal metaplasia on target biopsies by FICE according to Endo typing were 92%,86%,94%,respectively.【Conclusion】 FICE can not only capture the pptimal images of Barrett,S epithelium,but also show the pit pattern and capillary with good clinical value in targeted biopsy.
