米多君联合参附注射液治疗血液透析相关性低血压的临床观察

The Clinical Observation of Midodrine United with Shenfu Injection in Treating with Hemodialysis-related Hypopiesia

目的观察米多君联合参附注射液治疗血液透析相关性低血压的临床疗效。方法治疗组与对照组患者均于透析前30min口服盐酸米多君5mg,两组在统一透析条件下对每位入选患者分别连续观察其用药后10次透析前血压,透析过程中最低血压和透析后血压,患者出现IDH时,对照组降低超滤速度,提高透析液钠浓度,降低透析液温度,输注生理盐水或高渗葡萄糖等,治疗组给予参附注射液40mL静脉注射。观察两组治疗前后血压、心率的变化。结果治疗组患者共发生低血压62例次,经处理后显效43例,总有效率为91.94%;对照组共发生低血压65例次,经处理后显效31例,总有效率为69.23%,两组发生低血压患者经处理后,血压均有不同程度升高,治疗组效果明显优于对照组,且治疗后治疗组ALB、CRP与对照组比较有明显差异。结论米多君联合参附注射液应用起到协同作用,疗效明显优于单用米多君常规治疗,值得在临床上推广。

【Objective】 Observe a rice many gentlemans to unite the clinical curative effect that three remarks shoot the liquid treatment blood dialysis relativity low blood pressure.【Methods】 Treatment set and matched control sufferer all in dialysis the rice taking orally hydrochloric acid 30 minutes ago is many gentlemans 5 mgses,2 sets are 10 to is dialysis before the blood pressure after selecting a sufferer to distinguish to observe it to use a medicine in a row to each one under the unified dialysis condition,in the dialysis process the lowest blood pressure and the dialysis empress blood pressure,when the sufferer appeared IDH the matched control lowered super filtered speed,raised a dialysate sodium density,lowered dialysate temperature,lost to note physiology brine or Gao Shen glucose etc.,the treatment set gave three remarks to shoot the liquid 40 mL intravenous injection.Observe the variety of blood pressure in two sets of the front and back of treatments,heart rate.【Results】 The low blood pressure happens to treatment set sufferer totally 62 times,after processing show an effect 43,always efficient is 91.94%;The low blood pressure happens to matched control totally 65 times,after processing show an effect 31,always efficient is 69.23%,two sets of occurrence low blood pressure sufferer through processing after,the blood pressure all has different degree to heat up,the treatment set effect obviously surpasses a matched control,and the treatment set ALB,CRP and matched control relatively have obvious difference after the treatment.【Conclusion】 Rice many gentlemans uniting three remarks to shoot a liquid application have already been in conjunction with a function,the curative effect obviously better than list is used many gentleman normal regulationses of rices treatment,worth at clinical up expand.
