摘要
目的对比研究丙泊酚复合舒芬太尼静脉麻醉用于无痛人流术麻醉的安全性和有效性。方法 112例人工流产手术患者,随机分A、B二组,A组丙泊酚组,B组丙泊酚复合舒芬太尼组。并观察两组麻醉在无痛人流术不同时点对病人的BP、SpO2、HR、RR影响、术后镇痛效果。结果镇痛效果B组术中镇痛效果明显优于A组(P<0.01),术后宫缩痛明显轻于A组(P<0.05)。丙泊酚用量B组术中丙泊酚追加量和总量明显少于A组(P<0.01)。麻醉恢复时间B组麻醉完全清醒时间快于A组(P<0.01),离开恢复室时间明显少于A组(P<0.01)。术中术后不良反应两组术中皆有一定的呼吸抑制(SpO2<90%),但两组数据差异无统计学意义;术后两组皆有恶心呕吐及头晕头痛发生,但差异无统计学意义;两组术中给药皆有局部注射痛,B组明显优于A组(P<0.01)。结论舒芬太尼复合丙泊酚用于无痛人流手术的麻醉镇痛效果好、术中生命体征较平稳、呼吸抑制轻、术后恢复迅速,且能有效地抑制人工流产术后的子宫收缩痛,是增强丙泊酚无痛人流麻醉效果的最佳配伍。
【Objective】 To compare propofol intravenous anesthesia sufentanil anesthesia for painless surgery safety and effectiveness.【Methods】 112 cases of abortion patients were randomly divided into A,B two groups,A group of the propofol group,B group of propofol combined with sufentanil group.And anesthesia were observed at different time points in the pain of abortion patients BP,SpO2,HR,RR effect,analgesic effect.【Results】 of the analgesic effect of intraoperative analgesia in group B was significantly better than group A(P0.01),postoperative pain and contractions significantly lighter than Group A(P0.05).The amount of propofol in group B patients the amount of additional propofol and significantly less than the total group A(P0.01).Anesthesia recovery time in group B the time fully awake anesthesia faster than group A(P0.01),left the recovery room time was significantly less than group A(P0.01).Intraoperative and postoperative adverse events in both groups Jie certain respiratory depression(SpO2 90%),but no significant difference between two sets of data;postoperative nausea and vomiting and dizziness two Jieyou incidence of headache,but the difference statistically significant;both groups in the administration of local injection pain Jie,B group was significantly better than group A(P0.01).【Conclusion】 Sufentanil and propofol anesthesia for painless surgery analgesic effect of intraoperative vital signs were relatively stable,mild respiratory depression,postoperative recovery was rapid,and can effectively inhibit the contraction of the uterus after abortion pain,is to enhance the propofol anesthesia painless best compatibility.
