护理干预对男性留置尿管舒适度及并发症的影响

The influence of the nursing intervention on the comfort degree and complication of male patients with indwelling catheter

目的探讨护理干预对男性留置尿管患者的舒适度以及并发症的影响。方法收集2009年3月至2010年7月我科室留置尿管的102例男性患者，将其分为实验组51例和对照组51例。实验组将尿管固定在患者下腹部，以便于观察尿的颜色和尿量，导尿后每天清洁尿道口后予氯霉素眼水滴尿道口。注意插管操作，指导患者提肛肌等功能锻炼。对照组则进行常规无菌导尿护理。结果实验组发生尿路感染伴发热2例，拔管后尿道口粘连1例，膀胱刺激征6例。感到重度不适1例（2．0％），中度不适11例（21．6％），轻度不适19例（37．3％），无明显不适20例（39．2％）。实验组患者的尿路感染、尿道口粘连、膀胱刺激征等并发症发病率显著降低，舒适度明显较高。结论护理干预对男性留置尿管患者的舒适度有显著改善作用，可以减少并发症的发生，提高生活质量，值得临床推广。

Objective To investigate the infuence of nursing intervention on the comfort degree and comphcation of mate patients with indwelling catheter. Methods Between March 2009 and July 2010, 102 male patients detaining catheters in our department were divided into two groups ： experimental group and control group, 51 cases respectively. In experimental group, catheter was fixed to the lower abdomen for easy to observe the amount and color of urine. The urethral orifice was cleaned and then disinfected with chloramphenicol drops daily after catheterization. Functional training was instructed to patients. In control group, patients were cared with conventional way. Results In experimental group, 2 cases with urinary tract infection combined with fever, 1 case with meatus urinarius adhesions after removing catheter, 6 cases with bladder irritation. 1 case of patients felt severe discomfort （2.0%）, 11 cases with moderate discomfort （21.6%）, 19 cases with mild discomfort （37.3%）, 20 cases with no obvious discomfort （39. 2% ） . Complications of experimental group including unnary tract infection, meatus urinarius adhesious, bladder irritation, etc were decreased significantly, degree of comfort were improved evidently. Conclusions Nursing intervention could decrease morbidities and improve degree of comfort and the quality of life for patients in peri -- hospitalization, which is worthy of promotion.
