不同剂量瑞芬太尼用于妇科手术MAC的可行性研究 被引量:5

摘要 目的探讨瑞芬太尼用于妇科手术监测下的麻醉管理技术(MAC)的最佳剂量。方法将150例妇科手术患者随机分成3组,每组50例。在手术开始切皮前,各组皆缓慢静注瑞芬太尼0.25μg/kg,继之用微量泵持续输注瑞芬太尼,A、B、C组分别为0.03、0.05、0.07μg/(kg.min),于缝合皮肤时停止泵注。在不同时点监测记录平均动脉压(MAP)、HR、脉搏氧饱和度(SpO2)和呼吸抑制情况;观察记录术中患者对牵拉的反应级别,进行改良的OAA/S评分。结果各组间各时点HR、MAP、SpO2差异无统计学意义(P>0.05);改良的OAA/S评分差异有统计学意义(P<0.05),且呈明显的剂量依赖性。A、B、C组分别有0、3、18例患者出现呼吸抑制(P<0.05);A组与B、C组牵拉的反应级别有统计学差异(P<0.05),B、C组间无差异(P>0.05)。结论 0.05μg/(kg.min)瑞芬太尼泵注辅助应用于妇科手术硬膜外阻滞不全引起的镇痛、镇静不全和内脏牵拉反应效果确切、安全可行。
