那格列奈联合甘精胰岛素治疗2型糖尿病疗效观察

Therapeutic effect of nateglinide combined with insulin glargine on type 2 diabetes mellitus

目的以门冬胰岛素30注射液为对照,观察那格列奈联合甘精胰岛素治疗2型糖尿病的疗效和安全性。方法血糖控制不佳的2型糖尿病患者76例随机分为2组,每组38例,试验组予那格列奈联合甘精胰岛素治疗。对照组予门冬胰岛素30注射液治疗,2组疗程均为12周,观察治疗前后空腹血糖(FBG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbA1c)、体重指数(BMI)的变化以及不良反应的发生率。结果治疗后2组FBG、2hPG、HbA1c均较治疗前明显下降,差异有统计学意义(P<0.01),2组比较差异无统计学意义(P>0.05);试验组治疗后BMI与治疗前比较,差异无统计学意义(P>0.05),对照组治疗后BMI明显高于治疗前,差异有统计学意义(P<0.05),2组治疗后BMI比较,差异有统计学意义(P<0.05),试验组低血糖发生率13.2%(5/38),明显低于对照组47.4%(18/38)。结论那格列奈联合甘精胰岛素治疗2型糖尿病疗效显著,不增加体重,使用安全,患者依从性好。

Objective To observe the therapeutic effect and safety of nateglinide combined with insulin glargine on type 2 diabetes mellitus, as compared with those of insulin Hspart 30 injection. Methods 76 patients with type 2 diabetes mellitus whose blood glucose was not controlled well were randomly divided into two groups ,38 patients in trial group were treated with nateglinide combined with insulin glargine, however, the other 38 patients in control group were treated by insulin Hspart 30 injection,with a treatment course of 12 weeks for both groups. The blood sugar （ FBG）, 2-hour postprandial blood sugar （ 2hPG）, hemoglobin sugar （ HbA 1 c ）, weight index （ BMI ） were detected before and after treatment, and the incidence of adverse reaction was observed. Results After treatment the levels of FBG,2hPG, HbA1 c were significantly decreased, as compared with those before treatment in both groups（ P 〈 0. O1 ）,however, there was no significant difference between two groups （ P 〉 0.05）. After treatment there was no significant change in BMI in trial group, as compared with that before treatment （ P 〉 0.05）, however, which in control group was significantly higher than that before treatment （P 〈 0. 05 ）, furthermore, there was a significant difference in BMI between two groups （ P 〈 0. 05 ）. The incidence of hypoglycemia in trial group was 13.2% （5/38） ,which was significantly lower than that [47.4% （18/38）] in t control group. Conclusion The nateglinide combined with insulin glargine is effective in treating type 2 diabetes mellitus,with safety and good dependence for patients,without increasing body weight.
