摘要
目的探讨难治性癫痫患者智力、个性及心理健康状况,并分析对智能的影响因素。方法分别采用瑞文标准推理能力测验(SPM)、艾森克人格问卷(EPQ)以及症状自测量表(SCL-90)对68例难治性癫痫患者及60名健康者进行评测,并分析年龄、病程、发作情况等因素对难治性癫痫患者智力的影响。结果难治性癫痫患者的SPM总分及IQ值均较对照组低(P<0.05);存在智力缺陷的占51.4%,高于对照组1.7%(P<0.05);难治性癫痫患者EPQ各量表得分、个性分型分布与对照组无显著性差异(P>0.05)。难治性癫痫患者SCL-90的总分及躯体化、人际关系、抑郁、焦虑、敌对等因子分均高于对照组(P<0.05)。起病年龄、病程、发作类型、发作频率、发作持续时间、用药种数、SCL-90得分均与癫痫患者智力状况相关(P<0.05)。结论多数难治性癫痫患者的智力明显受损,并伴有不同程度的心理障碍。起病年龄、病程、发作类型、发作频率、发作持续时间、用药种数是难治性癫痫患者智力损害的重要影响因素。
Objective To explore the characteristic of intelligence,personality and mentality of patients with intractable epilepsy and an-alyze the related factors on intelligence.Methods 68 patients with intractable epilepsy and 60 healthy persons were assessed with Raven's standard progressive matrices(SPM),Eysenck personality questionnaire(EPQ) and 90 symptoms checklist(SCL-90).The related factors on intelligence were analyzed.Results Compared with controls,the total score and IQ of SPM of the patients decreased significantly(P〈0.05).There were 51.4% of intellectual deficiency in patients with intractable epilepsy,which was higher than that of controls(1.7%)(P〈0.05).There was no significant difference in score of EPQ and the proportion of personality type between 2 groups(P〈0.05).The total score of SCL-90 and the factor score,such as somatization,interpersonal sensitivity,depression,anxiety and hostility in patients were higher than those in controls(P〈0.05).Various factors,including the onset age,course of disease,seizure types,frequency,duration,number of drug,the scores of SCL-90 were correlated with the intelligence of epileptic patients(P〈0.05).Conclusion The intelligence of many patients with in-tractable epilepsy were damaged,and many patients were with various psychological barriers.The onset age,course of disease,seizure types,frequency,duration and number of drug were important factors related with intellectual disorders in patients with intractable epilepsy.
