杭州市浙大御跸社区高血压病规范化管理效果评价

The Effect Evaluation of Hypertension Standardized Management Techniques on Hangzhou City Yubi Community

目的评价高血压病规范化管理技术实施前后社区高血压病的管理效果。方法对2007—2009年浙大御跸社区高血压病人群的疾病管理情况进行评价,总结并改进社区高血压病规范化管理技术。结果通过3年的规范化管理,社区高血压病的控制率上升了4.00%,访视率上升了4.00%;高血压病患者总体收缩压实施前为146.14±14.781 mmHg,实施后为136.37±13.149 mmHg,舒张压实施前为82.80±9.613 mmHg,实施后为80.22±8.954 mmHg;结论高血压病规范化管理技术在社区高血压病管理中有着重要的作用,不仅提高了社区慢性病管理的水平,也促进了高血压病患者的健康。

OBJECTIVE Compare the management effectiveness before and after we used the hypertension standardized management techniques on Hangzhou City Yubi Community Health Service Station.METHODS Evaluated the effectiveness of standardized management techniques during the year 2007 and 2009,then summarized and improved it.RESULTS after three years,effectively control rate rise 4.00%,follow-up rate raise 4.00%;in 2007,the SBP of the hypertension people is 146.14±14.781 mmHg,in 2009,the SBP of the hypertension people is 136.37±13.149 mmHg,in 2007,the DBP of the hypertension people is 82.80±9.613 mmHg,in 2009,the SBP of the hypertension people is 80.22±8.954 mmHg.CONCLUSION The hypertension standardized management techniques plays an active role in community-based hypertension management.Not only improve the level of community chronic diseases management,but also to promote the health of hypertension patients.
