以精神障碍为突出表现的脑肿瘤──附22例分析

Brain tumor manifested prominently with mental symptoms: analysis of 22 cases

目的：探讨以精神障碍为首发或突出表现的脑肿瘤的临床特点。方法：对22例病人的临床资料进行回顾性分析。结果：大部分患者肿瘤位于间脑及颞叶，占73％（16／22），精神障碍症状以分裂样症状及痴呆表现为主。半数误诊为精神分裂症。结论：间脑及颞叶区肿瘤可以精神症状为首发或突出表现。对拟诊为精神分裂症、痴呆等精神疾患的患者应作头颅CT检查，以排除脑肿瘤。

Objective: To explore the clinica1 characteristics of brain tumor manifested initially or prominentlywith mental symptoms. Method: Clinical data of 22 cases were analyzed retrospectively. Results: Tumor withmental symptoms was mostly located in diencephalon or temporal lobe (73%, l6/22), schizophrenia and dementiawere the main manifestation found in these patients, half of whom were misdiagnosed as schizophrenia. Conclusion:Diencephalon and temporal tumor tends to manifest initially or prominently with mental symptoms. Diagnosis of men-tal disorders such as schizophrenia or dementia can only be made after exclusion of brain tumor by CT scan.
