急诊床旁彩超在异位妊娠破裂诊断中的应用价值

Application value of emergent bedside color ultrasonic examination in diagnosing ectopic pregnancy rupture

目的:探讨急诊床旁彩超在异位妊娠破裂诊断中的应用价值。方法:应用便携式彩色超声诊断仪对105例异位妊娠破裂的病人进行超声检查,分析其结果。结果:彩超诊断:附件区混合性包块伴腹腔积液(异位妊娠破裂并出血)97例,手术病理证实为输卵管妊娠63例,卵巢妊娠8例,腹腔妊娠26例。盆腔混合性包块5例,手术病理为腹腔妊娠。腹腔积液3例,手术病理为输卵管伞端妊娠。超声诊断准确率为92.37%。结论:急诊床旁彩超诊断快捷、无创、分辨力强、诊断特异性高,能为临床提供及时和切实可靠的诊断依据,应作为异位妊娠破裂诊断的首选检查。

Objective：To study the application value of emergent bedside color ultrasonic examination in the diagnosis of ectopic pregnancy rupture.Methods：The portable color ultrasonic diagnostic instrument was adopted to perform the ultrasonic examination in 105 cases of ectopic pregnancy rupture.The results were retrospectively analyzed.Results：Color ultrasonic diagnosis revealed 97 cases of mixed mass in adnexa area with seroperitoneum（ectopic pregnancy rupture and bleeding）.Among them,the surgical pathological examination verified 63 cases of fallopian tube pregnancy,8 cases of ovarian pregnancy and 26 cases of abdominal pregnancy.There were 5 cases of mixed mass in pelvic cavity,which were confirmed as abdominal pregnancy by surgical pathology.3 cases of seroperitoneum were verified as tubal fimbria end pregnancy by surgical pathology.The accuracy rate of ultrasonic diagnosis was 92.37%.Conclusion：Emergent bed side color ultrasonic examination is quick and noninvasive with high resolution and high specificity,which can provide timely and reliable diagnostic evidence for clinic and should be the first choice of examination for ectopic pregnancy rupture.
