摘要
目的 :如何确定Millard法中“5”点的正确位置 ,至关重要。作者试图采用几何投影原理以达到确定“5”点的目的。方法 :在健侧鼻底找出“5”点 ,在鼻小柱点定中P点 ,并通过该点作一条鼻小柱投影中轴线 ,该线与“3”—“5’”点联线相交 ,其相交点即为定点“5”点。结果临床共整复 86例 ,全部采用几何投影原理确定“5”点 ,随访 1- 3年 ,在75例获随访病例中 ,有 6 6例其中鼻小柱居中、鼻孔等大、鼻翼不塌陷和上唇高度相等方面获满意 ,成功率达 88%。结论 :要重视“5”点的正确定位 ,采用几何投影原理代替传统的定“5”点理论和方法可进一步提高单侧唇裂整复质量。
Objective: It's very important to correctly make point ‘5’ in Millard method. Authors have tried to mark point ‘5’ with geometric projection principle. Methods: Point ‘5’ has close relation to the projection line of the midline of the declivious collumella. while the projection line is made ,the point ‘5' changes regularly. According to this principle,locating point ‘5' in the base of the nose on the correct side and point ‘p' located in the midpoint of the columella ,then through point‘p' making a projecting midline to columella.this line cross to the line “3-5'”.The very crossing point is ideal point ‘5'. Results: 86cases of cleft lip had been operated with Millard method according the princple of Geometric Projection.Following time ranges from 1 to 3 years.Among those 75 cases had been followed up. Satisfaction in 6 cases who show equel size in nastrl, no deformity of alar, similar high of lip and very middle in columella. efficanty of operation is in 88 percent. Conclusions: It should be appreciated to correctly mark point `5'.This approach has been proved to be more advanced and scientific than the traditional methods. It's available to improve guality to repair cleft lip.
