内侧型蝶骨嵴脑膜瘤的显微手术

Microsurgical removal of medial sphenoid ridge meningiomas

目的 分析应用显微手术提高内侧型蝶骨嵴脑膜瘤全切除率的经验。 方法 回顾性分析 2 1例内侧型蝶骨嵴脑膜瘤显微手术治疗的资料。 结果 手术全切除 ( Simpson 、 级 ) 14例( 67% ) ,次全切除 5例 ,大部分切除 2例。死亡 1例 ( 4 .8% ) ,轻瘫 2例 ,失语 1例。 结论 大多数内侧型蝶骨嵴脑膜瘤与脑血管之间有一层蛛网膜界面相隔。在第一次手术中 ,应力争采用显微手术方法全切除肿瘤。

Objective To introduce the microsurgical experience of improving the rate of total removal in medial sphenoid ridge meningiomas Methods A series of 21 cases operated microsurgically on with vigorous attempts at total removal of the tumor were analysed retrospectively Result Total removal (Grade Ⅰ、Ⅱ of Simpson system)was achieved in 14(67%) of the 21 patients,and subtotal in 5,partial in 2 After operation,one patient(4 8%) died of pulmonary infection Postoperative paresis were seen in 2 patients,and aphasia in 1 Conclusion The interfacing arachnoid membranes is present between the tumor capsule and the cerebral vessels in most of the cases Microsurgical technique is recommended to totally remove the tumor with vigorous attempts during the initial operation
