摘要
目的: 评价CT对卵巢囊腺瘤的诊断价值。方法: 回顾性的分析了27 例经手术和病理证实的卵巢囊腺瘤, 其中浆液囊腺瘤13 例, 粘液囊腺瘤9 例, 交界性肿瘤5 例。全部病人均做了平扫后增强CT扫描。27 例共做了31 次妇产科US检查。我们着重分析各型肿瘤的CT表现特点, 并比较CT与US对本病的诊断效果。结果: 22 例浆液和粘液囊腺瘤, CT都诊断为良性卵巢囊腺瘤。31 次妇产科US检查, 28 次做了卵巢源性肿瘤的诊断。5 例交界性肿瘤, CT和US都未能做出准确的定性诊断。浆液囊腺瘤的平均大小为15cm, 平均CT值为12HU, 均为单房。粘液囊腺瘤的平均大小为19cm, 平均CT值为15HU, 9 例中7 例为多房。27 例中, 只有15 例CT定位诊断正确。31 次US检查, 28 次做出了准确的定位诊断。结论:CT是良性卵巢囊腺瘤影像诊断中十分可靠和有效的检查方法。但对于交界性肿瘤的定性诊断和体积较大肿瘤的准确定位诊断, CT仍有困难。
Objective:To evaluate the diagnostic value of CT in the examination of the ovarian cystadenoma.Methods:We reviewed and analyzed retrospectively the CT scans of 27 patients with ovarian cystadenoma confirmed by surgery and pathology,including 13 cases with serous cystadenoma,9 cases with mucious cystadenoma,and 5 cases with borderline tumor. All were examined with pre-and post-enhanced CT scanning and with ultrasound (US) of gynecological department in 31 cases. We analyzed with emphasis on lesion characteristics of CT findings and compare CT with US in detection of ovarian cystadenoma.Results:All ovarian tumors were unilateral.22 patients with the serous and the mucionous tumors were correctly diagnosed as ovarian benign cystadenoma by CT.US with 31 examinations permitted detection of the 29 tumors.5 patients with borderline neoplasma were incorrectly diagnosed as a benign tumors by CT and US.The serous cystadenomas were 15cm in mean size,12HU in mean value of attenuation, and unilocular lesion in all. The mucionous cystadenomas were 19cm in mean size,15HU in mean value of attenuation,and multilocular lesion in 7 of 9 the tumors. At CT,only 15 cases of 27 tumors were correctly demonstrated in localization. At 31 examinations with US,28 cases were correctly detected in localization. Conclusion:CT is the effective imaging modality in diagnosing ovarian cystadenoma.CT is difficul for determining pathologic nature of borderline cystadenoma and accurately location of huge tumors.
