经额底纵裂-终板入路切除颅咽管瘤

The operation experience of removal of craniopharyngioma via fronto-basal interhemispheric trans-lamina terminalis approach

目的探讨经额底纵裂-终板入路切除颅咽管瘤的手术方法和疗效。方法回顾性分析2008年10月—2010年10月18例经额底纵裂-终板入路手术的颅咽管瘤患者的临床资料。评价该手术入路、显微操作技术的优缺点。结果 18例均顺利完成手术,肿瘤全切除16例(88.9%),次全切除2例;保留垂体柄17例(94.4%),未保留1例。视力改善13例。术后并发症主要为多尿及电解质紊乱。结论经额底纵裂-终板入路是治疗颅咽管瘤安全有效的手术路径。

Objective To explore the method and effect of surgery via fronto-basal interhemispheric translamina terminalis approach for craniopharyngioma resection. Methods Clinical data of 18 patients who underwent surgery for craniopharyngioma via fronto-basal interhemispheric trans-lamina terminalis approach were analyzed retrospectively. Article mainly discussed in surgical approach and technique of microsurgery. Results The tumor was totally removed in 16 cases（88.9% ）, subtotally removed in 2 cases. The pituitary stalk was reserved in 17 cases（94.4% ）, and not reserved in 1 case. Visual improvement occurred in 13 patients, The main complications were diabetes insipidus and electrolyte disturbance. Conclusions The fronto-basal interhemispheric trans-lamina terminalis approach is safe and effective for removing craniopharyngiomas.
