蜂针疗法治疗类风湿关节炎膝关节滑膜炎的临床随机对照研究

Clinical Randomized Comparison Study of Bee-sting Therapy for Knee Synovitis caused by Rheumatoid Arthritis

目的:研究蜂针疗法对类风湿关节炎(RA)所致膝关节滑膜炎的临床疗效。方法:选取获得随访的RA合并膝关节滑膜炎患者46例。随机分为蜂针组和对照组。对照组采用MTX、SASP和美洛昔康片口服。蜂针组在对照组的基础上加用蜂针疗法,三个月为一疗程。治疗前后观察Lequesne指数、VAS、HSS评分和膝周长、B超积液和滑膜厚度探查、ESR、CRP检测。结果:蜂针组临床疗效优于对照组(P<0.05)。与对照组比较,蜂针组治疗1月、3月后Lequesne指数、VAS评分明显降低,HSS评分明显升高(P<0.05);膝周长和积液深度均明显减少。蜂针组、对照组与治疗前比较,1月、3月后Lequesne指数、VAS评分明显降低,HSS评分明显升高(P<0.05、P<0.01)。膝周长和积液深度、滑膜厚度均明显减轻(P<0.05、P<0.01)。结论:两组治疗RA所致膝滑膜炎均有较好的临床效果,但蜂针组见效早、疗效更为显著。

Objective：To study the clinical effect of bee-sting（venom）therapy to the knee synovitis caused by Rheumatoid Arthritis（RA）.Methods： forty-six RA combine with knee synovitis patients randomly divided into bee-venom group and medicine group,with 23 cases in each.Patients of medicine group were treated with oral administration of Methotrexate（MTX）,SASP and Meloxicam;and patients of bee-venom group were treated with Bee-sting on the basis of the above-mentioned Western medicines.The treatment lasted for 3 months.Observed the Lequesne index,VAS,HSS score,knee circumference,B–ultrasonic effusion,synovial thickness,ESR and CRP results before and after treatment.Results：Comparison between two groups showed that the clinical effect of bee-venom group is better than medicine group（P0.05）.Compared with medicine group,after 1 month and 3 months＇ treatment the Lequesne index,VAS score are obvious lower for bee-venom group,and also the HSS score is obvious higher（P0.05）,and the knee circumference and effusion depth are all reduced.Compared with pre-treatment,after 1 month and 3 months＇ treatment for both bee-venom group and medicine group,the Lequesne index,VAS score are both obvious lower,and the HSS score is obvious higher（P0.05,P0.01） too.The knee circumference,effusion depth and synovial thickness are also reduced（P0.05,P0.01）.Conclusion：Both of the above-mentioned treatments have good clinical effect to the knee synovitis caused by RA,but the bee-venom therapy have earlier and more significant effect than simply use medication.
