骨巨细胞瘤术中加热化疗的研究

Study of hyperthermic chemotherapy for giant cell tumor of bone during operation

目的 探讨骨巨细胞瘤切刮术中腔内加热化疗的方法及其对术后复发的影响。方法 肿瘤切除后，在手术腔内注满生理盐水，并加热至４３～４５ ℃，维持６０ ｍｉｎ ，后３０ ｍｉｎ 加入顺铂２００ ｍｇ。结果 ３０ 例随访２ ～６ 年。除１ 例术后１３ 个月复发截肢外（ 病理为Ⅱ～Ⅲ级），其余２９ 例无复发， 无转移及死亡。复发率为３－４４％ 。结论 术中病灶腔加热化疗，可在很大程度上杀伤残留的肿瘤细胞，进一步降低骨巨细胞瘤的术后复发率。

Objective To study the methods and effects of hyperthermic chemotherapy for giant cell tumor of bone during operation. Methods Solution was poured into the empty cavity after tumor was excised,then heating solution to 43～45℃ by a automatic constant temperature heater, continued for 1 h, and added cis platin 200 mg 30 min later. Results 30 cases were followed up for 2～6 years. There was only 1 case of recurrence,the result showed a lower recurrence rate of 3 44% without metastasis and death. Conclusion Remnant tumor cells may be destroyed by hyperthermic chemotherapy, and local recurrence of giant cell tumor of bone may be effectively prevented by the method.
