不同亚型胃食管反流病患者胃动力检测的研究

The study of gastric motility in sub-types of gastroesophageal reflux disease

目的:采用生物阻抗胃动力检测系统和评价方法研究不同亚型胃食管反流病(Gastroesophageal reflux disease,GERD)患者胃动力情况。方法:选取103例GERD患者,行GERD的反流症状(Gastroeso phageal reflux disease,GerdQ)评分,并通过胃动力检测系统提取胃电和胃阻抗信号,与对照组相比较,并进行不同亚型GERD之间胃电和胃阻抗信号的组间比较。结果:反流性食管炎(Reflux esophagitis,RE)组GerdQ评分为(12.52±8.49),非糜烂性反流病(Non-erosive reflux disease,NERD)组GerdQ评分为(15.14±8.01),Barrett食管(Barretts’esophagus,BE)组GerdQ评分为(7.20±7.10)。与对照组比较,不同亚型GERD的胃电和胃阻抗异常明显增加,差异有统计学意义(P<0.05)。RE组胃电和胃阻抗异常较NERD组、BE组更明显,NERD组胃阻抗中频百分比低于BE组,而低频百分比高于BE组(P<0.05)。NERD反流症状评分与胃阻抗异常相关r(s=-0.032)。结论:GERD各亚型均存在不同程度的胃动力异常,以RE组最明显。在NERD中反流症状与胃动力异常有关。

Objective：To investigate the situation of gastric motility by using bio-electrical impedance method among the subtypes of gastroesophageal reflux disease（GERD）.Methods：One hundred and three GERD outpatients were assigned to receive the GerdQ grade;impedance signal and electrical signal were detected and compared with the control group.Then the impedance signal and electrical signal among subtypes of GERD were compared.Results：The GerdQ score in RE was 12.52±8.49;NERD was 15.14±8.01;and BE was 7.20±7.10.The proportion of abnormal gastric motility in subtypes of GERD was higher than that in the control group.The proportion of abnormal gastric motility in RE was higher than in NERD and BE.The percentage of NERD was lower than the percentage of BE in median frequency of gastric impedance,but higher than that of BE in low frequency（P〈0.05）.There was negative correlation bewteen GerdQ and gastric impedance low frequency in NERD（rs=-0.032）.Conclusion：There is abnormal gastric motility in subtype GERD,especially in RE.The reflux symptoms score has correlation with gastric motility in NERD.
