高血压左心功能常规指标与声学定量技术比较

Comparison between conventional index and acoustic quantification technique of hypertensive left ventricular function

摘要 为了将反映高血压患者心功能的常规指标与声学定量技术(Acoustic Quantification,AQ)指标进行比较分析,评价高血压左心功能变化,将实验对象分为对照组30例和高血压病组45例,将高血压组分成非肥厚组和肥厚组;对各组进行了常规心功能指标及AQ检测。各组间DBP、LVM、LVMI、IVS、IVPW、LA有显著性差异(P<0.05)。与对照组相比,非肥厚组与肥厚组的VE/VA及RF/AF降低,AFF及LVESV增加(P<0.05)。与对照组相比,肥厚组RFF降低,PAFR、LVEDV及OAFV增加(P<0.05)。常规心功能指标中EF同AQ指标中EF进行非配对T检验,显示前者低于后者(P<0.05)。实验结果表明,AQ技术作为一种新的能评价舒张功能障碍的新方法,可视为常规超声心动图指标的补充,结合此两种方法可有效提高临床诊断左心室舒张功能异常。 This study compared the acoustic quantification(acoustic quantification,AQ) with the conventional echocar-diographic parameters,to evaluate the changes of the left ventricular function in patients with essential hypertension.A total of 30 control subjects,45 patients with essential hypertension(EH) including patients without left ventricular hypertrophy(N-LVH) and patients with left ventricular hypertrophy(LVH) were divided into 3 groups.The three groups were measured by conventional echocardiographic parameters and AQ technique.DBP,LVM,LVMI,IVS,IVPW and LA showed significantly different among the three groups(p0.05).Compared with control subjects,the VE/VA and RF/AF in N-LVH group and LVH group were decreased,AFF and LVESV in N-LVH group and LVH group were higher(p0.05).Compared with control subjects,RFF in LVH group was lower,PAFR,LVEDV and OAFV in LVH group was higher.The non-paired T test was adopted to compare EF in the conventional echocardiographic indices and EF in the AQ index,the result showed that the latter EF was lower than the former one,and the difference was statistically significant(p0.05).As a new assessment method of diastolic dysfunction,AQ echnology can be regarded as a comple-ment to the traditional Doppler echocardiography,combining with the two methods can effectively improve the clinical diagnosis of left ventricular diastolic dysfunction.
