早期负重锻炼对非骨水泥全髋关节置换术后假体周围骨密度的影响 被引量:2

摘要 目的探讨早期锻炼对全髋关节置换术后假体周围骨密度的影响。方法2009年12月-2010年10月收治46例老年髋关节骨性关节炎患者行全髋关节置换手术治疗,术后随机分为早期负重锻炼组和非早期负重锻炼组,分别给予康复锻炼,所有患者进行平均14个月随访.随访时进行Harris髋关节评分,每个患者在术后1周、3个月、6个月和12个月进行骨密度检查。结果两组患者Harris髋关节评分无显著差异,所有患者术后12个月内股骨假体周围骨密度均有显著降低,早期负重锻炼组术后3、6个月股骨近端骨密度显著高于非早期负重锻炼组,术后12个月两组骨密度无显著差异。结论股骨假体周围骨密度反映假体周围骨丢失情况,早期负重锻炼可减缓全髋关节置换术后假体周围的骨丢失,提高术后患者生活康复质量。 Objective To explore effects to periprosthetic bone density by early weight-bearing exercise for patients after non-cemented total hip replacement. Methods Between December 2009 and October 2010, 46 patients with hip osteoarthrities treated by non-cemented total hip replacement were randomly divivided into early weight-bearing excerse group and non-early weight-bearing excerse group after operation. All patients were followed up average 14 monthes and scored of evaluation according to Harris score criteria. Periprosthetic bone density evaluation were performed on all patients at lweek, 3 month, 6 month and 12 month postoperatively. Results There was no statistics singnificence of Harris score beteween the two groups, and perithetic femoral bone density of every patient was decreased singnificently at 3,6 month postoperatively. The proximal femoral bone density of patients in early weight-bearing exercise group were more higher than those of patients in non- early weight-bearing exercise group, but there no statistic significence between the two groups at 12 months postoperatively.Conclusions Early weight-bearing exercise after total hip replacement can prohibit periprosthetic bone lose, and improve live qulity of postoperative rehabilitation.
