Prognosis traits of elderly outpatients with depression in cognitive-behavior therapy and influencing factors

摘要 目的分析接受门诊认知行为治疗的老年抑郁症患者的预后特征及其有关影响因素。方法使用海斯曼心理CT系统和汉密顿抑郁量表对56名接受门诊认知行为治疗的老年抑郁症患者进行测试,评估其预后特征、家庭问题、个性和心理特点等。结果 (1)女性的疾病预后不良、心理预后不良分数高于男性(P<0.05),回归生活能力分数低于男性(P<0.05);认知行为治疗疗效好者疾病预后不良和心理预后不良分数低于疗效差者(P<0.01),康复动力和回归生活能力分数高于疗效差者(P<0.01)。(2)多元逐步回归分析显示,个性特点、抑郁、适应不良、控制力、支配性、依赖性、自我强度和性别是预后特征的相关影响因素(P<0.05)。结论接受门诊认知行为治疗的老年抑郁症患者的预后特征受抑郁症状、个性特点、适应不良和自我强度等心理特征的影响,可以预示临床疗效。 Objective To study the prognosis traits of the elderly outpatient with depression in the cognitive-behavior therapy and the influencing factors.Methods A total of 56 elderly outpatients with depression in the cognitive-behavior therapy were investigated by applying Minnesota Multiphasic Personality Inventory(MMPI) and Hamilton Depression Rating Scale for Depression(HRSD).Results(1) The female outpatients had higher scores of disease poor prognosis and mental poor prognosis than the male(P0.05),and had lower scores of re-employment ability than the male(P0.05).The outpatients getting better in the treatment had lower scores of disease poor prognosis and mental poor prognosis(P0.01),and had higher scores of re-employment ability and rehabilitation power than the worse patients(P0.01).(2) The prognosis traits of the elderly outpatients were remarkably predicted by depression,personality,ego strength,maladjustment,controlling force,dominant trait,dependency and gender(P0.05).Conclusions The prognosis traits of the elderly outpatients with depression in the cognitive-behavior therapy are affected by depression,personality,maladjustment,ego strength,etc.and in accord with the therapeutic effects.
