双源CT检查无症状糖尿病患者冠脉病变的应用价值 被引量:2

Value of dual-source CT angiography in evaluation of coronary lesion in patients with asymptomatic diabetes

摘要 目的探讨双源CT血管成像(DSCTA)对无症状糖尿病(DM)患者冠状动脉病变的应用价值。方法对108例无症状患者(DM组52例,非DM组56例)行冠状动脉DSCTA,显示每例患者的病变血管节段,并区分是否为阻塞性病变(管腔狭窄≥50%),分析冠状动脉狭窄程度及斑块性质,并比较DM组与非DM组患者的斑块特征。结果 DM组冠状动脉病变发生率比非DM组高(P<0.01),DM组受累的冠状动脉节段数明显比非DM组多(P<0.01),DM组3支病变明显多于非DM组(P<0.01),DM组闭塞病变明显多于非DM组(P<0.01),DM组弥漫性病变明显多于非DM组(P<0.01),DM组钙化计分比非DM组高(P<0.01)。结论 DSCTA方便、快捷、无创,是评价DM患者冠状动脉病变的首选检查。 Objective To explore the value of dual-source CT angiography(DSCTA) in the evaluation of coronary lesion in patients with asymptomatic diabetes mellitus(DM).Methods DSCTA was performed in 108 patients without known coronary artery disease(CAD) including 52 diabetics(DM group) and 56 non-diabetic(non-DM group) patients.Diseased coronary segments of each patient were displayed,and each diseased segment was classified as obstructive lesion(luminal narrowing ≥ 50%) or not.In addition,plaque type(non-calcified,calcified,and mixed) was analyzed,and plaque characteristics were compared between the two groups.Results DM patients had a higher prevalence of coronary lesion,more diseased coronary segments,a higher prevalence of three-vessel lesion,more obstructive lesion and diffuse lesion as well as a higher calcification scoring than non-DM ones(P0.01).Conclusion DSCTA can well demonstrate the location,degree and plaque characteristics of coronary lesion.It is convenient,rapid,non-invasive,and therefore can be taken as the first choice of examination for the evaluation of coronary lesion.
