骨肉瘤、骨软骨瘤与正常人血清蛋白指纹图谱分析

The analysis of proteomic profiling in serum samples of patients with osteosarcoma and osteochondroma and normal volunteers

摘要 目的分析骨肉瘤和骨软骨瘤患者血清蛋白指纹图谱的差异,筛选对骨肉瘤早期诊断有意义的特异性蛋白。方法收集河南省洛阳正骨医院2007年6月至2010年1月病理确诊的骨肉瘤25例、单发骨软骨瘤16例和性别、年龄与上两组患者匹配的26例健康志愿者,采用SELDI-TOF-MS质谱仪方法对这67例静脉血液离心获得的血清进行蛋白指纹图谱分析。结果我们发现2个特异性蛋白质荷比(M/Z)具有差异性表达,即3954Da、6438Da。在学习模式下其诊断灵敏度96.0%,特异度96.1%,在测试模式下其诊断灵敏度98.5%,特异度100.0%,采用leave-one-out互证交叉检验验证,认为该决策树盲法筛选分组的正确率可达67/67=100%,灵敏度为25/25=100%;特异性为42/42=100%。结论采用SELDI-TOF-MS方法筛选出的2个特异性蛋白可能对骨肉瘤的早期诊断和治疗监测有临床参考价值。 Objective To figure out the difference between proteomic profilings of patients with osteosarcoma (OS) and osteochondroma (PC) and normal volunteers and identify OS-related specifc proteins for early-stage diagnosis. Methods From June 2007 to January 2010, 25 OS patients, 16 solitary PC patients confirmed by pathological diagnosis and 26 age-matched normal volunteers (as reference) were gathered up in Luoyang hospital of orthopedics and traumatology. Surface-enhanced laser desorption/ionization-time-of-flight mass spectrometry (SELDI-TOF-MS) was applied to perform proteomic profiling of all 67 venous serum samples. Results Two statistically significant proteins (M/Zs were 3954Da and 6438Da) were found to be differentially expressed in OS serum samples when compared with those of other groups. The sensitivity and specificity of diagnosis were 96.0% and 96.1% on learning mode, while on test mode were 98.5% and 100.0%. With proteomic profiles and external leave-one-out cross-validation, it was showed that the correct rate of allocation, the sensitivity and specificity of diagnosis were 100%. Conclusions The 2 specific proteins identified by SELDI-TOF-MS may be clinically helpful to early-stage diagnosis and therapeutical monitoring of OS.
