Syndecan-1与VCAM-1在多发性骨髓瘤中的表达及意义探讨 被引量:1

摘要 目的本研究旨在探讨多聚体蛋白聚糖(Syndecan-1)和血管细胞黏附分子-1(VCAM-1)在MM患者骨髓单个核细胞(BMMCs)中表达水平,初步探讨Syndecan-1和VCAM-1对MM的发生、发展及预后的意义。方法收集45例MM患者及10例正常对照的骨髓标本,获取BMMCs,采用实时荧光定量PCR(Real time PCR)的方法检测BMMCs中Syndecan-1和VCAM-1 mRNA的表达,并同步抽取血浆,检测β2微球蛋白(β2-MG)分泌水平,按照Durie-Salmon(D-S)分期标准对MM患者进行临床分期。结果 (1)Syndecan-1和VCAM-1 mRNA在MM组中表达升高,与对照组比较差异有统计学意义,治疗后缓解或平台期Syndecan-1 mRNA表达水平明显减低(P<0.05);(2)复发/难治性多发性骨髓瘤患者Syndecan-1和VCAM-1 mRNA的表达水平较正常对照组及平台期患者明显升高(P<0.05),初发组与复发/难治组之间表达无显著差异。Syndecan-1和VCAM-1 mRNA表达与患者病程D-S分期和β2-MG水平有相关性。结论 MM患者Syndecan-1和VCAM-1 mRNA表达水平升高,并且MM初治组、复发/难治组Syndecan-1和VCAM-1 mRNA表达水平均高于平台组,提示Syndecan-1和VCAM-1可能参与MM的发生、发展的病理机制;Syndecan-1和VCAM-1 mRNA表达水平与MM肿瘤负荷呈正相关,说明Syndecan-1和VCAM-1可以作为评判MM患者预后的新指标。 Objective To explore the expression levels of Syndecan-1 and VCAM-1 in patients with multiplemyeloma (MM) and their significance on occurrence, development and prognosis of MM. Methods Bone marrow specimens from 45 patients with MM and 10 normal person were collected to obtain BMMCs. Real-time polymerase chain reaction (Real time PCR) was applied to detect the expression of Syndecan-1 and VCAM-1 mRNA. At the same time, plasma samples were taken to detect the secretion level of β2-MG, according to which MM patients were staged by Durie-Salmon (D-S) staging system. Results The expressions of Syndecan-1 and VCAM-1 mRNA in patients with newly diagnosed and relapsed/refractory multiple myeloma increased markedly, which had a significant difference with the control group. The expression level of Syndecan-1 mRNA markedly decreased in patients MM reaching clinical remission (CR) or plateau stage (P〈0.05). Compared with normal control group and groups in CR or plateau stage, the expression levels of Syndecan-1 and VCAM-1 mRNA in patients with relapsed/refractory MM increased markedly (P〈0.05). There was no statistically significant difference in expression between newly diagnosed and relapsed/ refractory patients. The expressions of Syndecan-1 and VCAM-1 mRNA were related to the stage of disease and β2- MG level. Conclusions The expression levels of Syndecan-1 and VCAM-1 mRNA in patients with newly diagnosed or relapsed/refractory MM increase, which are higher than those of patients in plateau stage, indicating that Syndecan-1 and VCAM-1 may be important factors for the pathogenesis of MM. Tumor burden is positively correlated with Syndecan-1 and VCAM-1 mRNA, which indicates that they can be regarded as new indicators for the prognosis of MM.
