摘要
目的:探讨胃癌前病变组织中人端粒酶逆转录酶(hTERT)和p53蛋白的表达状况及其与幽门螺杆菌(Hp)根除前后的关系。方法:采用实时定量荧光RT-PCR方法检测胃粘膜中hTERT,将hTERT与GAPDH(甘油醛-3-磷酸脱氢酶)拷贝数之比的100倍作为标准化hTERT(NhTERT),用快速尿素酶法和Warthin-Starry银染色法检测Hp,同时用免疫组化检测p53蛋白的表达。结果:Hp阳性组hTERTmRNA和p53蛋白的表达率均显著高于Hp阴性亚组的表达率(均P<0.05)。Hp根除组hTERTmRNA的表达率显著低于未根除组(P<0.05),未根除组与阴性对照组之间无显著性差异。结论:在胃癌前病变阶段,Hp感染与端粒酶表达水平及p53蛋白之间有直接关系,提示端粒酶、Hp感染、p53蛋白在胃癌的发生、发展中起重要作用及Hp根除的必要性。
Objective:To investigate the expression of human telomerase reverse transcriptase(hTERT)and p53 in human gastric precancerous lesions and to explore the association of hTERT expression and p53 with helicobacter pylori(Hp)infection.Methods:Real-time quantitative RT-PCR was used to quantify hTERT mRNA.The copy number of hTERT was normalized by GAPDH transcripts as internal control and expressed as 100×(hTERT/GAPDH) ratio.Hp was detected by rapid urease test and Warthin-Starry silver staining.The method of immunohistochemical to assay p53 protein.Results:Compared with atrophic gastritis without Hp infection,the expression level of hTERT and p53 in atrophic gastritis with intestinal metaplasia was significantly higher(P0.05).The expression level of hTERT and p53 in Hp eradication group was significantly lower than Hp positive group(P0.05).Expression of hTERT was no significant difference between no eradication and negitive control group.Conclusion:Hp infection rate is directly related to hTERT and p53 expression level in gastric precancerous lesions,suggesting that telomerase and Hp infection may play an important role in the genesis and progression of gastric carcinoma.
