摘要
目的测定国人不同年龄段的膝关节软骨MR T2-mapping成像中的T2弛豫时间值,探讨膝关节软骨的生理演变规律。资料与方法选取100名10~69岁行膝关节MRI检查的志愿者,入选标准为无膝关节病史及手术史,无明显临床相关症状,MRI上膝关节软骨无形态及信号异常表现。每10岁为一个年龄跨度,每组15~23名志愿者。运用关节软骨T2-mapping成像技术测量各年龄组膝关节髌骨、股骨远端及胫骨内外侧髁关节软骨的T2弛豫时间值,进行组间统计学比较分析。结果 (1)髌软骨T2弛豫时间值10~19岁年龄组与30~69岁各年龄组差异均有统计学意义(P<0.05);20~69岁各年龄组中,10~19岁与20~29岁组之间差异无明显统计学意义(P>0.05);(2)30岁以上年龄组内,胫骨平台内侧及外侧软骨T2弛豫时间值差异有统计学意义(P<0.05);(3)10~19岁与40~69岁各年龄组,股骨远端与胫骨平台内侧及外侧软骨T2弛豫时间值差异有统计学意义(P<0.05),10~19岁与20~39岁各年龄组之间差异无统计学意义(P>0.05);(4)各年龄组中,股骨远端与胫骨平台内侧及外侧软骨T2弛豫时间值差异有统计学意义(P<0.05),股骨远端软骨T2弛豫时间值高于胫骨平台内侧及外侧软骨;(5)30岁以上各年龄组间,股骨远端与胫骨平台内侧及外侧软骨T2弛豫时间值之间差异无统计学意义(P>0.05)。结论通过有限人群的不同年龄段膝关节软骨T2弛豫时间值测定,初步可以在形态学改变前做为参考标准评价早期软骨病变,同时也可以作为定量参考指标用于评价软骨有无损伤以及治疗后随访评估,具有一定的临床价值,但有待于进一步研究和完善补充。
Objective To measure the T2 relaxation time of normal knee articular cartilage in different age.Materials and Methods One hundred healthy adults(10-70 years old) underwent MR T2 mapping examination.The cases were divided into six groups,with ten years gap in each group and 15 to 23 cases in each group.The T2 relaxation time values in patella cartilage,distal of femoral cartilage,medial and lateral platform of tibial cartilage were measured.Statistical differences were analyzed between each group.Results(1) There was statistical difference of T2 relaxation time values in patella cartilage between 10-19 years group and 30-69 yeasrs groups;(2)T2 relaxation time values had statistical difference between medial and lateral platform of tibial cartilage in 30-69 years groups;(3) There was no statistical difference of T2 relaxation time values in distal of femoral cartilage,medial and lateral platform of tibial cartilage between 10-19 group and 20-39 years group,but there was statistical difference between 10-19 and 40-69 years groups;(4) There was statistical difference in T2 relaxation time values between distal of femoral cartilage,medial and lateral platform of tibial cartilage in each group.T2 relaxation time values in distal of femoral cartilage was higher than that of medial and lateral platform of tibial cartilage;(5) There was no statistical difference of T2 relaxation time values in distal of femoral cartilage,medial and lateral platform of tibial cartilage between 30 years groups.Conclusion T2 relation time of normal knee cartilage in different age can be used as a standard to evaluate early pathological changes in osteoarthritis,and it has important value in follow up and evaluation of clinical treatment.
