摘要
目的探讨右心室心尖部及流出道两部位起搏与电张调整性T波的相关性。方法动态观察72例安装R波抑制型起搏器(VVI)或房室万能型起搏器(DDD)患者心电图的T波,其中右心室心尖部起搏58例,右心室流出道起搏14例。结果 58例右心室心尖部起搏患者均出现电张调整性T波,且在下壁、前间壁导联明显;14例右心室流出道起搏患者均未出现电张调整性T波。结论右心室心尖部起搏产生的电张调整性T波有其独有的规律性,动态观察有助于与心肌缺血鉴别。
Objective To explore the correlations between different pacing locations,the apex and the outflow tract,and the electrotonic modulated T wave.Methods Electrotonic modulated T waves of 72 cases with VVI or DDD pacemaker were observed dynamically.Of these cases,58 paced from the apex of the right ventricle and 14 paced from the outflow tract of the right ventricle.Results Electrotonic modulated T waves were observed in all of the 58 cases pacing from the apex of the right ventricle,especially in leads of the inferior wall and the anteroseptal wall.No electrotonic modulated T wave was observed in the 14 cases pacing from outflow tract of the right ventricular.Conclusion Electrotonic modulated T waves paced from the apex of the right ventricle exhibit unique regularity which is helpful for differential diagnosis from myocardial ischemia when observed dynamically.
