连续性非卧床腹膜透析患者1个月后血压及残余肾功能变化研究 被引量:6

Changes of Blood Pressure and Residual Renal Function after One Month of Continuous Ambulatory Peritoneal Dialysis

摘要 目的观察连续性非卧床腹膜透析(CAPD)患者治疗1个月后的血压、残余肾功能及尿量的变化。方法研究对象为2007—2009年在广西医科大学第一附属医院肾内科初次行腹膜透析管安置术后进行CAPD治疗的终末期肾脏病患者129例,于CAPD治疗1个月末时评估患者的透析充分性,腹透前及CAPD 1个月末时观察患者的血压、尿量、估算肾小球滤过率(eGFR)及相关生化指标。结果患者CAPD 1个月末时透析充分性良好,尿素氮、肌酐水平较透析前降低(P<0.05),血红蛋白较透析前增高(P<0.05)。收缩压、舒张压较透析前降低(P<0.05)。eGFR较腹透前略有降低,但差异无统计学意义(P>0.05)。尿量较腹透前稍有减少,但差异亦无统计学意义(P>0.05)。结论 CAPD在透析早期疗效显著,透析充分性良好,患者营养状况好转,高血压有所控制,残余肾功能下降不明显,患者尿量减少不明显。 Objective To observe the changes of blood pressure, residual renal function, and urine volume after con- tinued ambulatory peritoneal dialysis (CAPD) of one month. Methods A total of 129 new peritoneal dialysis patients lander went CAPD at our hospital between Jan. 2007 and Dec. 2009 were included. Biochemical indices and blood pressure, urine vol- ume, and estimated glomerular filtration rate were observed before dialysis ; adequacy of dialysis was evaluated after one month of treatment. Results Dialysis adequacy was satisfiable among CAP]) patients after one month of treatment, levels of serum urea and creatinine decreased significantly ( P 〈 0. 05 ), and levels of haemoglobin increased significantly ( P 〈 0. 05 ). Systolic blood pressure and diastolic blood pressure decreased significantly after dialysis ( P 〈 0.05 ). Levels of eGFR and urine volume decreased without statistical significance ( P 〉 O. 05 ). Conclusion Our results suggest that CAPD improves the condition of the patients at the early stage of dialysis. Adequacy of dialysis was satisfiable and nutritional status was improved among CAPD pa- tients. The hypertension status was also improved. Decrease of residual renal function and urine volume was slight.
