无创正压通气治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭的疗效分析

Noninvasive positive pressure ventilation in the treatment of chronic obstructive pulmonary disease with type II respiratory failure effect

目的：观察无创正压通气治疗慢性阻塞性肺病（COPD）合并Ⅱ型呼吸衰竭临床效果。方法：将73例COPD合并Ⅱ型呼吸衰竭患者随机分为两组。对照组（33例）给予常规治疗，观察组（40例）加用无创正压通气治疗；比较两组治疗前后的动脉血气分析、呼吸频率、心率、血压、Borg评分、插管率及住院天数。结果：两组在治疗后动脉血气pH值、PaCO2、PaO2、心率和呼吸频率与治疗前比较均有明显改善（P〈0．05），观察组在降低PaCO2、提高PaO2、减少插管、缩短住院时间方面比对照组效果更明显（P〈0．01）。结论：无创正压通气有利于COPD合并Ⅱ型呼吸衰竭患者改善通气及生命体征，缓解痛苦，缩短住院时间。

Objective Observation of nonlnvasive positive pressure ventilation in the treatment of chronic obstructive pulmonary disease （ COPD ） with type Ⅱ respiratory failure clinical effect. Methods 73 cases of COPD with type Ⅱ respiratory failure patients were randomly divided into two groups, the control group （ 33 cases ） was given routine treatment, The observation group （ 40 cases ） with noninvasive positive pressure ventilation therapy; comparison of two groups before and after treatment of arterial blood gas analysis, respiratory rate, heart rate, blood pressure, Borg score, intubation rate and length of hospital stay. Results Two groups in treatment of arterial blood gas PaCO2 , pH value, PaO2, heart rate and breathing rate compared with before treatment will be improved obviously （P 〈 0.05）, the observation group in the lower PaCO2, improve PaO2 , reduce intubation, shorter hospital stay effect more apparent than in the control group （ P 〈 0. 01 ）. Conclusion Noninvasive positive pressure ventilation in favor of COPD combined with type Ⅱ respiratory failure patients improved ventilation and vital signs, Relieve the pain, shorten the time of hospitalization.
