经皮骨水泥椎体成形术治疗单纯椎体压缩性骨折体会 被引量:1

摘要 目的探讨经皮椎体成形术(PVP)治疗单纯椎体压缩性骨折的疗效。方法在C形臂X线机引导下,局部麻醉下,对29例单纯椎体压缩性骨折的37个椎体通过经皮、椎弓根途径行椎体成形术,其中29个椎体采用带球囊的骨水泥填充,8个椎体采用非球囊骨水泥填充,观察患者术后疼痛缓解情况及压缩椎体高度恢复情况。结果 29例患者术后麻醉药药效结束后,疼痛均得到有效的控制,术后2小时后可佩戴腰围下地活动,一般患者次日即可出院,按时返院随诊。术前平均痛觉视觉模拟评分(VAS)评分平均为8.1分,术后VAS评分平均为2.4分。复查X线片可见采用球囊型骨水泥扩张、填充的患者椎体高度恢复较为满意,术前压缩椎体前、中缘高度相当于后缘高度的57.91%,术后高度恢复到89.75%,无骨水泥渗漏到椎体前缘或椎管现象;采用非球囊型骨水泥填充的病椎高度恢复稍欠理想,术前压缩椎体前、中缘高度相当于后缘高度的67.13%,术后高度恢复到75.36%,有三例少量骨水泥渗漏到椎体前缘,无渗漏到椎管现象。结论经皮椎弓根椎体成形术治疗单纯椎体压缩性骨折是一种有效、安全、缓解疼痛快的方法,不但是大家易于接受的微创手术,且避免了长期卧床所带来的各种并发症,提高患者术后生活质量,是值得推广的手术方法。
